PTSA Membership Application - More than 30 years of research has proven that children do better when their parents are involved both at home and in school.   Please become an active PTSA member and see for yourself how rewarding it can be.  Depending on your interests and available time, you can join one of our committees, attend monthly meetings, or assist at PTSA events.  It all begins with membership.  For only $5 you can become a member of the PTSA for 2011-2012!

CMS Student Directory Sign-up 2011-2012 - Order your copy of the Student Directory for only $5.  The form is also used to include a student on the “Do Not Publish” list in the directory or to request a student to not be included.  All other students will automatically be in the directory.  Deadline extended until October 14th.

2011-12 Mini Grant Application -  The PTSA is once again pleased to offer the Mini-Grant Program at CMS.  The program is open to individual teachers, departments, teams or student organizations.  If you have a project or program you would like to offer students and need financial assistance, we would like to hear from you.  The CMS PTSA has budgeted $4,000 for the 2011-12 school year for the purpose of mini grants to be able to cover many different programs in the school.    Please complete this form and return it to Mr. Dalton by December 2, 2011.  The PTSA officers will review applications as they are received and determine the status of requests on a rolling basis.

Privacy Letter  - State regulation and district practices require that the school administration ask parental permission to use student information in district publications, PTA/PTSA directories, local newspapers and magazines, and on the district web site and cable stations.  If you haven’t done so already, complete the form and return to your child’s school.
